1. The Background


 A private multi-specialist hospital was seeing its revenue drop by 15% despite having "clean" financial books. Management’s instinct was to cut costs by reducing nurse overtime. I was brought in to look beyond the ledger and find the real reason for the leak.

  • The Thesis: You cannot manage a hospital with an Income Statement alone. You need to manage the Patient Experience and Staff Health.

2. The Discovery


 I spent a week observing the hospital flow. And I found two major leaks that didn't have their own line on the Balance Sheet.

Leak #1: The Patient Exit Tax (Wait Times)

  • The Data: Average wait time was 150 minutes. Cross-referencing sign-in sheets with billing, I found that 20% of walk-in patients left before their consultation.
  • The Math: 10 patients lost daily  ₦20,000 avg. bill = ₦200,000 lost revenue/day.

    • Annual Impact: ₦200,000  365 = ₦73,000,000 in lost revenue.

Leak #2: The Sunk Cost of Turnover (Nurse Retention)

Management saw "Salaries" as the cost. I showed them the Replacement Cost

  • The Math (Per Nurse):

    • Separation/Agency Cover: ₦100k
    • Recruitment/Interviewing: ₦150k
    • Training/Learning Curve: ₦250k
    • Total: ₦500k per exit.
  • Annual Impact: 40 nurses leaving/year \times ₦500k = ₦20,000,000 in sunk costs.

3. The Solution


 I designed a Managerial Dashboard that tracks the "Health" of the business in real-time.

4. The Managerial Pivot


 I explained to the Board that by spending ₦5,000,000 on a digital queue system and a "Retention Bonus" for nurses, they weren't increasing expenses, they were investing in flow.

The Logic Chain: Better Nurse Morale  Faster Patient Care \rightarrow Lower Wait Times \rightarrow Stop the ₦73M Leak.

5. Final Recommendation & Result


Recommendation: Implement the Non-Financial Dashboard and prioritize Nurse Retention over cost-cutting. The Result: Within 4 months, wait times dropped to 50 minutes. The hospital recovered ₦12M in "lost" revenue in the first quarter alone, effectively paying for the new system 2x over.
